We use a case management system to ensure effective use of professional resources to achieve positive patient outcomes. Each patient is assigned a care team leader, usually a registered nurse or registered physical therapist, who will manage your care. Your care team will then take the time to educate you about the plan of care outlined by your doctor, and they will periodically assess your progress and report any changes to your physician.
Medicare, Medicaid or private insurance reimburses the services provided by the Home Health Agency, depending on the level of care and your insurance plan.
