Our trained staff, which includes a cardiac nurse specialist, physical therapists, occupational therapists, speech therapists, home care aides and a social worker, provides individualized care with the goal of keeping our patients out of the hospital and functioning at the highest level possible. Although our patients suffer from a variety of heart-related infirmities, we treat the entire person, not just the heart-related illness.
Our cardiac program provides:
- An individualized congestive heart failure program with specific protocols that meet our patients’ medical needs
- Medication and disease process education
- Access to a telemonitoring tool that ensures our patients are seen when they most need a visit
- In-home infusions of medications, such as Lasix, Bumex, Doputamine
- Close working relationships with doctors, which includes giving updates as needed and sharing information from the telehealth machines.
