Full and part-time associates who work 20 or more hours per week are eligible to participate the first day of the month following 30 days of employment.

- Preventive Care – plan pays 100%
- Basic Care – plan pays 80%
- Major Care – plan pays 50%
- Orthodontia (for children under 19) –
Plan pays 50%, up to $1,000 per child per lifetime.
