Full- and part-time and part-time associates who work 20 or more hours per week are eligible to participate the first day of the month following 30 days of employment.
Delta Dental of Missouri – Discounted provider fees in-network (PPO and Premier networks) Annual maximum benefit of $1,500 per covered person.
- Preventive Care – plan pays 100% (both Premier and PPO networks)
After a $50 calendar year deductible:
- Basic Care – plan pays 80% (Premier) or 85% (PPO)
- Major Care – plan pays 50% (Premier) or 55% (PPO)
Orthodontia (for children under 19) – Plan pays 50%
up to $2,000 per child per lifetime (both Premier and PPO networks)
